Privacy policy
1. Who is responsible?
Dr. L. Stofferis BV, Sperredreef 20, 8890 Moorslede (company number BE 0794.281.134) is the data controller for personal data processed at the practice and through this website. For privacy questions, contact dokter.stofferis@outlook.com or 056 50 97 99.
2. Data collected through this website
This website has no contact form or user accounts, and uses no analytics, tracking or advertising services. The website's hosting provider does record technical data, such as your IP address, browser type and time of visit, in server logs. This keeps the website secure and working properly (legitimate interest, Art. 6(1)(f) GDPR). These logs are not used to identify you.
3. Booking online
Online appointments are handled by external booking systems: Dr Stofferis's online booking system, and the Rosa platform for appointments with Ymke Decat. The data you enter there is processed by those providers under their own privacy policies, to schedule your appointment.
4. Data related to your care
If you are a patient, or contact us by phone or email, we process your identification and contact details and your health data. We do this to provide care and keep your patient record. The legal bases are the physician's legal obligations (Art. 6(1)(c) GDPR) and the provision of health care (Art. 9(2)(h) GDPR), under the Belgian Patients' Rights Act of 22 August 2002 and the Act of 22 April 2019 on the quality of healthcare practice.
Your data is shared only when necessary for your care or required by law, for example with other healthcare providers involved in your treatment, or with your health insurance fund through the secure eHealth and MyCareNet networks. Everyone with access to your data is bound by professional confidentiality.
5. How long we keep your data
Your patient record is kept for at least 30 years and at most 50 years after the last contact, as required by the Act of 22 April 2019. Other data, such as emails without medical content, is kept no longer than necessary.
6. Your rights
You have the right of access, rectification, erasure (insofar as the legal retention obligation allows), restriction of processing, data portability and objection. As a patient, you also have the right to consult and obtain a copy of your patient record under the Patients' Rights Act. You can exercise your rights via dokter.stofferis@outlook.com or at the practice. We may ask you to prove your identity.
If you disagree with how we process your data, you can lodge a complaint with the Belgian Data Protection Authority, Rue de la Presse 35, 1000 Brussels (dataprotectionauthority.be).
7. Security
We take appropriate technical and organisational measures to protect your data against loss and unauthorised access. Please avoid sending sensitive medical information by ordinary email; discuss it during your consultation instead.
8. Changes
This privacy policy may be updated. The latest version is always available on this page.
